KVSC Patient Information

Health Insurance

We offer state-of-the-art treatment for patients who have rheumatoid arthritis, lupus,
osteoarthritis, and many other autoimmune diseases, as well as osteoporosis.

Talk to your health insurance company before you make an appointment to make sure your visit will be covered under your health plan. If we participate in your insurance plan, we will submit claims for you.

Referrals, Copays and Payment Policies

Before your doctor visit, find out if you need a referral and what your copay, if any, will be. All referrals and copays are due at the time of your visit. If you do not have your copay or referral, your appointment will be rescheduled.

If we do not participate in your insurance plan, payment is expected at the time of service. We will submit covered laboratory and X-ray services to your insurance company for you. Any services deemed non-covered or deductibles are your responsibility.

Even though we may participate or are in-network with your insurance, there is no guarantee their payment will be accepted as payment in full. Some plans, depending on the procedures billed, may have co-insurance and/or deductibles the patient is responsible for. Talk to your health insurance company first if you have any questions about health plan costs or coverage.
